Stellar black holes are made when the centre of a massive star falls in upon itself or collapses. When this happens, it causes a supernova. A supernova is an exploding star that blasts part of the star into space. Scientists think supermassive black holes were made at the same time as the galaxy they are in (NASA, 2018).

In addition to Siti’s answer, a black hole might also be formed when two neutron stars (super dense stars created when stars weren’t quite big enough to make black holes) get close and merge together.
This process of neutron star mergers is so intense it’s one of the only events we have successfully observed gravitational waves from https://en.wikipedia.org/wiki/Neutron_star_merger
